N4 Connect

Community Care

The Community Care program supports people with intellectual and developmental disabilities, dementia, and related conditions, as well as their family care partners.

Serving Washoe, Storey, Carson, Douglas, Lyon, Churchill, Pershing, Humboldt, Lander, Eureka, Elko and White Pine counties.
